  • Bank Account Number & Routing Number for Direct Deposits or IRS/STATE Due Payments

Returning clients - $170

First time or renewal - $130

Must bring:

  • Valid Passport

For kids:

  • Original birth certificate (additional fee is translation is requested)
  • Vaccine report (with ordinal signature & date)
  • School letter
