  • Last tax return
  • ID
  • Original Social Security Card (or Tax ID Card) for each individual
  • Bank Account Number & Routing Number for Direct Deposits or IRS/STATE Due Payments

Must bring:

  • Valid Passport

For kids:

  • Original birth certificate (additional fee if translation is requested)
  • Vaccine report (with ordinal signature & date)
  • School letter

Unregistered businesses, sole proprietors, sub-contractors, 1099 workers and single member LLC’s.
